Every time an officer had to fire his/her weapon during shift a report must be filed

was the rule in the police department where I worked as a dispatcher years ago. You had to fire your gun, you had to do a report about what happened and why.

I guess that's not the case in police departments anymore?

my experience has it that whenever an officer made a "contact" (i.e., a less than probable cause stop), a report was filed. But as you mention, that must not be the case in MO, or on Fox.

A cop acquaintence accidently discharged his gun while unloading it to show a mutual friend during lunch. Shot a hole in the floor. He had to fill out an extensive report.
