re: ACA. Who will suffer, and how?

This questions pertains to the ACA as it exists today compared to the pre-ACA status quo, or compared to how Republicans would "reform" the ACA. (Not compared to a better plan. It can be granted as a tautology that a better plan would be better.)

Which consumers (aka patients) suffer under the ACA? Who pays more or receives less in services?

What is the dimension of their hardship, if any?

(I would be surprised if anyone suffered a great deal, but I am curious what anyone upset about the ACA is upset about as it pertains to individual consumers/patients.)
