A pair of earphones that are comfy and don't fall out?
Is that too much to ask for?

1. Nope
Here you go: http://www.amazon.com/exec/obidos/tg/detail/-/B000095SB6/qid=1114570924/sr=52-1/ref=sr_52_1_etk-accessories/103-8954017-0517411?v=glance&s=electronics&n=281407

I'd suggest getting them off ebay, you'll save about $10 if you're lucky. They rock, but they do go in your ear, if that bothers you.

4. I have a pair of these
They sound pretty good and are a comfortable fit. They stay in place well and have 3 different sizes of rubber cushions for different size earoles.
Good value if you buy them from Amazon.

2. There are some available, but they're not cheap.
Prices range from ~$40 to over $200(!!!). Sony & Bose might be good makes to check out. They come with different sized inserts, and some can fit a custom hearing aid mold. There are even models with external noise cancelling electronics.

I too have had that same problem, for as long as "earbuds" came packaged with portable audio equipment. The design of my own ears appear to be "unique", and NONE of those freebies were more than barely useful to me.

6. I can't get earbuds to fit comfortably, either
For times when I may be outdoors & working up a sweat, I have a $20 pair from Sony that go around the back of my head, with the tiny speakers resting just outside the ear canal. Got it at RadioShack, I think.

Otherwise, I prefer my old Sony MDR-V6's, the kind that encloses the entire ear. Literally the best-fitting, most comfortable headphones I ever wore -- and I've owned them for over fifteen years. Unfortunately, the foam pads began falling apart. Some fortuitous Googling about six months ago, however, led me to an audio afficionado website, where they recommended velour-covered replacements from some sort of studio-quality 'phones. Now they're just like new!

3. I have small ears. I purchased the sony noice cancellation
headphones. There are 2 pairs. I got the ones that go in the ear. They come with removable ear doo hickeys in pairs of 3 sizes. I use the small.
Pricey but worth it.

5. I've found that the only type that don't fall out for me...
...are the kind that have the hook that wraps around your ear.

7. The ipod earphones don't fit my ears...ears too small
A friend of mine recommended EAR JAMS. Ordered them.

8. A friend (major audiophile) just showed me a pair that...
look just like those rubber ear plugs you buy in k-mart (or wherever), but they're wired for sound. Not only do they allow you to listen to music but they block outside noise. Supposedly they sound great. Very pricey though ($100+).

9. I have some like that, made by Phillips.
They were pricey, but they are pretty comfortable. I don't think they were $100+ though. More like $60-70.

11. I probably should have gotten the Sony earbuds... ended up with Nike
Every other pair of buds I've tried has fallen out while I'm biking (which is a really big PITA). The Nike ones have a thin strip that goes around the back of the neck and, unlike some of the cheaper models, the cord also fastens in the back.

The guy at the store promised me that if I didn't like them, I could exchange them for the Sony. We'll see.
