In order for your insurance company to approve the surgery, you probably have to have a history of back pain directly related to your breast size.
I'm large-busted too, and after my son was born I had ballooned to a 38-G. The weight of my breasts seemed unbearable. My gyn asked me about discomfort and pain, and made a very specific note in my medical chart that I had back pain related to breast size, and told me she was doing this on the chance that I might be interested in breast reduction surgery later in life. I was grateful for her foresight. I'm down to a 36DD now, and my breasts seem much lighter than their post-partum days, but I'd love to go down to a C-cup some day.
It's my understanding that "pre-existing" conditions only come into question if you've actually been treated for it before. So, if you haven't already been dealing with this medically, you're in the clear.
A friend of mine had the surgery when she was 31, and though the recovery was difficult, she loved the results.
