My thought for a Halloween costume this evening
I was thinking of putting on a suit and handing out these to people.

Notice of Denial of Coverage
Dear former plan member:

Regarding your recent submission of claims, we must inform you that we are
unable to provide coverage to you due to a pre-existing condition.

According to medical records obtained from your physician, you have been
deemed to be found alive. Since life is a terminal condition and known to be
fatal, and since the finding of life predates the onset of your recent medical
condition, we must deny your claim.

If you have any questions or wish to speak to a claims agent, please call our toll free number 888.xxx.xxxx twenty four hours a day. Please be aware, however,
that calling this number may be considered evidence of the aforementioned preexisting condition, (life), and may be used as evidence against you in any upcoming legal actions this company may be taking against you with regard to insurance fraud.

4. Well, living in DC , that wouldn't be scary...
...but I suppose I could take the train into Virginia and walk around with it there. Then I would scare a few people!
