*****NOTE: TIME SENSITIVE You must go in person to your voter registrar office BEFORE July 14! I just did and there were a half dozen others doing the same. A $5 charge, but worth it imo. The clerk typed the info in as I stood there and shredded the form I filled out. ********

I'm going to the Jeffco clerk and recorder's office on Monday.

ETA: for Jeffco the location for the forms is 3500 Illinois Suite 1100.
6th ave to the Jeffco county campus and on to Illinois and the Laramie Building.

4. Did they make you list your reasons to qualify?

6. You have to sign that you have reason to believe that you or your family could be at risk of

"harassment" (i don't remember the exact language), but it was vague enough that I don't think most would have an issue with truthfully certifying that they do.
