I do hold out hope that saner folks will prevail. We already see the intel community withholding information from this loser of a president, thus it is hopeful that career military brass will step up and refuse the orders of a madman.
One can only hope ( while shopping for home outside the US).

In most cases, no. That would be a coup, which they are not going to do.

If it were to nuke an American city, pretty good chance. That's also not likely to do.

As far as nuking NK, I'm sure that top military leaders would be in on the decision-making process. He isn't going to do anything without their say-so.
