Originally used to describe anybody who was acting to conceal the identity of someone else.

A woman that a gay man dates or marries to hide the fact of his true sexual orientation.

Or, it can also refer to a single man, that another (married) man, is using to hide the fact he his cheating on his spouse. Joe Kennedy Sr. had many affairs with other women (Gloria Swanson was one) during his lifetime. He was always seen in public with these other woman, but he had another man along he could claim was her "real" date.

5. It is a, hopefully, now somewhat outdated concept.

Within my lifetime (born 1984) being openly gay has become a viable option for far more people than before. Not that plenty of guys aren't still closeted - especially in more conservative areas - but the number is probably less and less as the years go by.
