We have a lot of bikes here (I live near Rotterdam). All my family ride bikes and so does just about everybody I know, even the car drivers. So, the awareness level is very high. So much so that it becomes pretty automatic.
Also, if you simply open your door, you'll very probably not have just one cyclist running into you. It could be quite a few. And then you suddenly have 20 very disgruntled cyclists showing their lack of gruntle from extremely close range.
