Some people may be anatomically and physically male or female, but gender dysphoria causes them to not recognize themselves in their own body. It can be completely a psychological condition.

My best friend is a trans male, born totally female, and said he could not even look in the mirror at himself until he begun transitioning because nothing made sense.

He actually got into cutting before he was diagnosed because he literally wanted out of the skin that didn't match who he was.
