My mom started having trouble signing anything on a line, when her Alzheimer's first started to become apparent. Her vision was fine. She simply could not put the tip of a pen on a line, and sign her name.

Later that spread to losing her perception of distance and her 'hand to mouth' function. Every meal had me guiding the fork into the food and then towards her mouth.
