The source was a sheriff's deputy, who is also reliable. Hospitals are pros at covering up for these kinds of mistakes and screw ups. Rather than accept this as a mistake on Clinton's part, the story needs to be fully investigated.

In this day and age, women supposedly under the care of obstetricians don't just up and die along with their newborns. Something went terribly wrong in the health care provided to this woman.

And while she may or may not have been "insured", demanding cash payment up front for co-pays is a growing practice and could have led to a barrier to timely care. Blaming the patient for their own death, while convenient for the hospital, isn't right and seldom stands up under scrutiny.
