Several years ago, I had a flaky DVR that would stop working until you rebooted it.

After the third time it did so, I called and requested a replacement box, since a DVR that stops is not reliable-enough. They tried to convince me that since it was working AT THE MOMENT, they could not replace it, and was there "anything else we can do to continue to provide you with excellent service." I had to argue my way up three levels of management, threatening to cancel and fight them on the termination fee in court if necessary, before they replaced the box.

For the last couple of years since then, it has been OK, except for the fact that they keep SPAMMING my DVR with unwanted promos for other options. At times, the tuner has changed channels in order to record this advertising. And my emails asking for them to turn it off have gone unanswered.

I think there is FIOS in my future.
