My bank (a local credit union) required me to fill out the paperwork before they would credit the charges. They also wouldn't even begin to process the paperwork until the charges were actually posted to my account. Also my bank reduces my available balance even before the charges are posted, so my balance was reduced on Friday evening, but it took until Tuesday before the charges actually appeared (my bank is notoriously slow about such things). However on Saturday they knew who the charges were from and we determined they were fraudulent. On Tuesday when the charges actually posted, they processed the paperwork, but it took until Thursday until my account was reimbursed.

The biggest hassle was the loss of my debit card, which took over two weeks to get replaced. Fortunately my wife's debit card has a different number so we didn't have to cancel that one.
