Where can i get a new voters card???

Where can i get a new voters card???
mine isn't looking too good and I'm scared that they wont except it.. i tried to go on registration sites to get a new card but it only gives me options of "new address, new name " and something else... but none to get a new card...
HELP

3. Here ya go, it has it all, its a pdf application, scroll down to page two.

There is a list of all Election commission offices with address's and phone numbers. You can call and ask them how to go about getting a replacement card. Good luck.
