Attention Web Site design Gurus. I need help with an on line question form.

I'm trying to design for my site a page similar to this one.

I need my customers to answer questions that I ask them and I need those questions to be delivered to me in some manner via email, etc.

I really don't know how to Google for info on this?

2. you need some backend scripting language to send the email

Something written in Perl or ASP or something. What server software are you using???

You also have to name your form variables correctly.

3. I desined my page with Dreamweaver

I can use Dreamweaver OK--- enough to make an OK website. BUT beyond that I need an expert. How do I find one?

7. Use the help files in Dreamweaver

they'll explain how to use the 'forms' section of the object manager to design your form. Then your domain needs something like sendmail to implement the form and feed the results to you.

Or, lookup Coffee Cup flash form maker...no fuss, no muss, nice forms.

6. Do you have the "For Dummies" book?

I did mine in GoLive and they have a way of creatitng a form. It's pretty easy. Then you have to contact your webhost to make it work. It's not that hard.
