Has anyone ever gotten access to your checking account - not talking of someone you know

I listen to Clark Howard on occasion (tv is so bad these days), and a caller was telling how unauthorized purchases appeared on her checking account, some coming from the US and others coming out of another country. She did not say it was a debit card or anything like that.

How does that happen, and has it happened to anyone else?

1. That happened to me once.

I don't even know what the company was selling. Some telemarketing company that had somehow gotten enough of my information to charge me straight to my bank account... even though they'd never called me before. I don't know if it was a mistake and they signed up the wrong name on their calling list or if it was an actual scam/purposeful theft, but either way I got my money back.

The big problem was that the transfer back took a couple of business days... and I discovered this when I was trying to pay my RENT that month. I totally should have sued or something.
