Or is anyone's avatar fair-game, even if they created it themselves?

1. That would be rude in my view, yes
The thing is, there is no need to do so. Just ask them for permission. Most will be flattered that someone liked it enough to ask, and consent.

Someone ripped mine off, and I must say that my initial reaction was not pleasant.

2. Yeah, my reaction was pretty negative
To be fair, I'm not using my calvin right now. but I plan on going back to it when the primary season is over (or if Edwards endorses Hillary).

It was a bit jarring to see someone else poaching the avatar I spent two days creating.

5. If it's really custom and they use it in the forum they found it on ....
then I'd say yes. They should ask before using in that case.

I have a bunch of Milla Jovovich, and a few Katie Sackhoff, avatars I can share if you're interested.

My folders are messy but I'm pretty sure I have them somewhere.

We had a member of the Skeptics group (Mr. Blur) create a whole bunch of custom avatars for us. In fact, I'm using one of them! Of course he said they were free for use. I suppose asking is the best policy.:)
