2. Whenever we housetrained a puppy -
the house always smelled like a French Hoor-house for two weeks. Wherever puppy went that he or she was not supposed to, we cleaned it up and then sprayed the cheap Xmas perfume our relatives always sent us (mainly "Windsong" and "Emerude") on that spot.
The puppy no longer associated that spot with "a place to go", and quickly learned, (with other supporting methods like praise) that outside was the place to go.
We didn't crate train, either - in those days, that was something breaders did. And since most of our dogs were larger than 20 lbs by the time they were 6 months, newspaper training was rather silly - they would not be left in the house for more than 3 hours on their own anyway.

5. Crate training, stick them outside the second they start to sniff and...
get something like Urine-B-Gone to neutralize the odor of their accidents.
