3. I think that might be a southern thing.

I seem to recall being called "Ms. (First Name)" while living in the south. I think it is done intentionally but as a means of being very polite. FWIW.

I have a dear, dear friend who will not allow her daughter to call me just Cathy, but instead of telling her to call me Mrs. F, she taught her to call me Miss Cathy. I don't say anything because it's not worth hurting my friend's feelings, but it DOES grate.
