I actually looked up the order.

5. Here's the GF's allegations (threatened to kill her)

"The Defendant wanted me to call him at a certain time on the 14th, when I didn't call at that time he became angry. I went to his house to see him and he began to argue with me and call me names. Wehn I argued back he grabbed me by my hair. and said "do you think I'm going to let you talk to me like that, I don't let anyone talk to me like that". He dragged me to the stairs and said that he would kill me if he ever found out that I dating any of his friends and that he if saw my car he would throw a brick through the window. I was able to leave at that point"

He said that a gun is buried in the park near his house.
