10. Maybe I've just watched too many episodes of the West Wing...
But Toby Ziegler (the fictional communications director) arguably had more power than the lower level cabinet members let alone their chiefs of staff. I recall one episode where he calls the Secretary of HHS into his office and bitches him out for planning to announce something that Toby wanted announced from the White House.

I know that in real life the senior staff are far more numerous than on that show and thus the Communications Director doesn't wield that kind of influence. But Chief of Staff to the Commerce Secretary really seems like a step down.
