...it was "Blue Train." In that photo, Coltrane's index finger is horizontal, curled over his lip, as if he were concentrating, perhaps listening to a playback in the studio; not vertical, as is usually the case in the "Shhh" gesture.

You might be thinking of a fairly widely-circulated photo of Miles Davis, from late in his career. However, I don't think it was ever used as an album cover. Here's a version of it:
