The Imperial Sugar Refinery explosion, which was caused by explosive dust particles which ignited could have been prevented. Right now there is no OSHA regulation on the books regarding explosive dust particles, there are only recommendations in which companies can choose to adhere to on a voluntary basis. The Imperial Sugar Company chose not to bother. The death toll is now at 12 dead and 11 still in critical condition due to wounds from the explosion.
