Underlings speak when spoken to and unrequested "lip" is insubordination. While not having been in uniform for quite some years, he seems to have brought that dismissal of dissenting voices with him to the Senate. In addition, McCain's right of passage to the presidency has been honored in this military manner on the Republican side and is unused to the messy, less hierarchical process of the Democratic party.
