I'll speculate that it has to do with the quality of the client.

3. The women are more attractive--often college students.

The perspective I offered here is just from having worked in the entertainment industry, where the excess is equal if not far worse. The $40 mil figure is clearly the high/high end of the business. But even someone making a million or two a year can afford to splurge on a top-end call girl once a month. Some actors who I won't name did this every week, or more frequently. Spitzer utilized these services approximately eight times over the course of "more than a year," according to the stories.
