Or, are the people who hold others accountable--the regulators--always one step behind?

1. how many years did Madoff screw over his investors?
even if regulators are behind, they don't have to be 20 years behind.

I'd be happy if they caught the criminals after the first offense.

7. There was no regulation. That's the problem.
Roll back all the regulations to where they were in 1980. Trade agreements (and tax brackets)too, but that's entirely another thread.
