I was curious about this as it could apply in 2020.
Apparently it’s only happened once to an elected president, Franklin Pierce (14) in 1856. Others ( John Tyler, Millard Fillmore, Andrew Johnson and Chester Arthur) were denied nomination but were not elected to the presidency and all had assumed the presidency as VP at the death of the president.

1. It'll be very unlikely.
Trump is unpopular - but very popular with the base, the people who'll renominate him.

2. I had a great great etc.... grandson of Franklin Pierce for my 5th grade teacher
He pretty much said that his esteemed ancestor was most known for being one of the worst presidents ever.
Also, that they both had 6 toes on one of their feet (or both feet? I forget). He then proceeded to show us that foot. Probably one of the few things I remember from school that year, LOL. |

4. I think you are forgetting LBJ
if I'm reading your criteria correctly. While he was VP in 1963 when JFK was killed, LBJ won the nomination on his own in 1964, but didn't run in 1968 because of poor poll numbers.
