Stand normally. You generally lean backwards. Or at least straight up. Up normally the natural backbone shape takes effect.

Now stand on your toes. You have to lean forward. At least I do. Because the backbone reacts to the position of your legs.

8. I tend to believe it's more to do with trying terribly hard not to look so fat.

Almost all of the cameras, especially video, are almost squarely front, and focused on the torso/shoulders/head instead of the whole body. The forward lean brings his shoulders and head more into line with his gigantic gut, creating an illusion of reduced rotundness.

Trump hates being fat, more than most of us because his vanity is complete. But, alas, the man is a walking avatar for every last one of the seven deadly sins and his gluttony gets in the way of his pride.

I'm sure he fakes his height, but millions of women wear heels every day without looking like, well, that thing.
