NTSB faults Air Canada pilots for last year's near disaster

Federal safety officials blame two Air Canada pilots for coming within 10 to 20 feet of crashing their jetliner into a plane on the ground last year in San Francisco.
The Air Canada pilots were apparently confused because one of two parallel runways was closed and dark before the late-night incident. The crew was seconds from landing their Airbus A320 jet on a taxiway where other planes loaded with passengers were waiting to take off.
We could not have gotten literally or figuratively any closer to having a major disaster, said the safety boards vice chairman, Bruce Landsberg, during a hearing Tuesday in Washington.
Underscoring the severity of the incident, the NTSBs top aviation-safety staffer, John DeLisi, said it was the first time the board considered a major investigation for an event in which there were no injuries or damaged planes.

(26,326 posts)Then another row of lights immediately this side of the water. Thats the other planes lined up on the taxiway.
The taxiway (and the runways) ends at the water line at screen center right. The Air Canada plane comes in over the bay high and in the center of the screen . You can see its landing lights reflecting on the bay as it approaches.
The Air Canada plane over flies the aircraft on the taxiway. The AC landing lights even reflect on the ground between two of the standing aircraft at one point.

In the go around . The aircraft directly to the left of my circle has illuminated his landing lights to warn the approaching AC aircraft. The tail emblem lights are visible on the next airplanes in line

And agree that would have likely involved several aircraft and bee a massive loss of life.
People went home and hugged their loved ones. I am sure.
Just wow! I had no idea it was THAT close a call.

(405 posts)This near disaster could have been avoided if the crew tuned the ILS frequency for the correct runway into the FMS, which is standard practice for most airlines.

(3,286 posts)....the pilot had been awake for 19 hours. As an old truck driver, I know the effects of fatigue and this was a pretty good example.
