The same things you get in a $56 dildo versus a $10 dildo.

Ergonomic shaping, better materials (silicone, metal, glass, polished unfinished wood...versus rubber), better construction (cheap products are often injection-molded which leaves a seam where the molds come apart), vibration, movement or other stimulation.

For $2000 you can get a solid gold buttplug. For the person who has too much money and wants to jam it in their fanny.

Any $10 sex toy you buy is virtually guaranteed to be made of rubber softened with phthalates, which are now widely recognized (even by the FDA) as a potentially serious carcinogen. They probably don't hurt much when they're used in your shower curtain or in the seals around your car door, but you don't want to be drinking or eating anything off of phthalate bearing dishes, and you CERTAINLY don't want to be inserting them into your various orifices. In spite of that, the web and adult stores are riddled with cheap sex toys made with the stuff (hint: if you pull it out of the package and it smells "plasticky" even after a rinse, you can be 95% sure that it contains phthalates).

Quality sex toys are phthalate free, so you don't have to worry about your favorite buttplug giving you sphincter cancer.

or they sell really expensive gear. $56 will buy you a very nice butt-plug but market-rate is usually closer to $20.

Some people like the feeling of fullness they provide.

Some like the anal stimulation.

Some men enjoy the way certain oddly-shaped ones stimulate the prostate.

They also have use in certain BDSM scenes where, when used in combination with a restraint that fits under the clothes, they function as a means of bowel-movement denial. (Some people get off on not being allowed by their dominant to use the bathroom.)
