I don't have nor have I ever had an iPhone 4.
I tried rebooting, it went away, but then came back.
Not gonna lie, I am a little freaked out

show app or show original file
lets see where this is from
if you find it, do a Get Info

Go to System Settings > Apple ID
Then have a look at the devices list. If there are any devices you do not recognise then select it and remove from your account.
