4. It was on Route 67 in Ballston Spa (NY). I remember reading it was listed as in need of repair.

But I'm naturally scared of bridges. I sometimes have to cross the Thaddeus Kosciusko Bridge (also known as the Twin Bridges) over the Mohawk River north of Albany - and I always try to take an alternate route.
