you overlook the universality of the rule that slow traffic keep right. Though the left lane may not be "the fast lane" the right lane is definitely the slow lane, so it becomes sort of a word game at that point.
In all instances, a vehicle driving slower than the "flow of traffic" should be in the right lane except when preparing to turn left.
I disagree with your rigid analysis, but agree with the general statement that on a 4-lane road with a lot of opportunities to turn left where there are no left turn lanes nobody should expect the left lane to be "the fast lane"
