"Privileged, pampered, and pointless."
In contrast to people like Whitney Houston who gave us a body of work which was beautiful and enduring, what did he do for the world? Take?
I like the story, "The Journey of Ibn Fattouma", written by Nobel-prize winning novelist, Naguib Mahfouz. To paraphrase a bit of dialogue in the first section, the protagonist (Fattouma's son of the title) asks the local imam, "What good is Islam when so many people are sick, hungry, poor, needy amongst the incredible wealth of a society with members who claim to be muslim, are incredibly wealthy but miserly and do nothing about any of these problems?"

Happened to a kid (14-16) at my high school. Not drug or extreme weight related.

Could be that, or rheumatic fever(does anyone still get that?). Those are possibilities.

True. The fact that this was a royal family of great wealth, however, is what prompts the question.
