Not perfect, but a lot of states are doing their best. There is probably some bureau of vital statistics at a federal level which reports all deaths. By looking at excess deaths this year over the numbers last year, you get a somewhat crude but reasonably accurate number of Covid-19 deaths.

In some countries, a lot of deaths are simply not reported at all. The people are just buried. I rather suspect that's true in at least some parts of Mexico, and from what I understand is very true of India.

While we are still not testing enough people, we're doing a semi-reasonable job of it, especially compared to a lot of other countries, like Mexico and India. I look at this every single day, and it's very enlightening. https://www.worldometers.info/coronavirus/#countries

Keep in mind that a hundred years on epidemiologists are still arguing over just how many people died of the 1918 flu. The true numbers of this pandemic will never be fully known, and a lot of guesswork will take place.
