I was wearing Smartwool socks, a pair of thermal tights, a long sleeve jersey and a windbreaker.

My feet had still been getting cold, even with shoe covers, because with clipless pedals, you still have a metal connector to the exposed pedals, which makes a great heat sink.

What I found is that sticking a pair of these under the insole of the shoes does the trick:
