I'm a knitter/crocheter. If it were me, I would make something that takes time and effort, something that shows my appreciation for the therapeutic relationship you've built. Maybe a soft, warm blanket she could use in her office on the back of a chair?

I'm not sure jewelry, as suggested above, is quite right - despite that you've known her for many years, it is still a professional relationship, as it should be. Jewelry is a little bit intimate, maybe boundary-crossing.
